living at the service. People in care homes receive accommodation and personal care as a single package under one contractual agreement. CQC regulates both the premises, and the care provided. Both were looked at during this assessment.

This assessment was completed due to concerns raised by the local authority and the volume of notifications submitted to CQC around the quality of care and leadership.

People did not consistently receive safe care and treatment, risks were not always identified, assessed or managed in a timely or effective way. Some incidents we found had caused distress to people.

We found environmental safety concerns were significant, including worn and dirty carpets, unsafe storage of mobility equipment, and fire safety risks, such as an outdated risk assessment and unclear evacuation routes.

Medicines were not always managed safely. Staff administered “as needed” medicines without documented reasons, and care plans lacked essential guidance. Some people were offered safe medicines support, and relatives described positive interactions with nursing staff, but this was not consistent across the home.

Staffing levels and deployment of staff around the home did not ensure people’s safety. People and relatives reported delays in response to call bells, particularly at night, and concerns about agency staff consistency. One resident said staff “never have enough time to spend with you, they are always short‑staffed.” Although some staff interactions were caring and respectful, this did not mitigate the impact of staffing shortages. Infection prevention and control was not consistently effective, with inspectors observing dirty equipment and poor ventilation.

Leadership did not ensure systems for monitoring safety were reliable or acted on promptly. The service was found in breach of legal regulations relating to safe care and treatment and governance at the last inspection. We found the home remained in breach of these regulations with a further breach in safeguarding identified. While some improvements were made during the assessment period, these did not provide sufficient assurance that people consistently received safe care.
