We identified 5 breaches of legal regulation in relation to safe environments, staffing, person centred care, consent and governance. The provider did not have effective governance systems in place to monitor and improve the quality of the service. The service did not always ensure recommendations to learn and improve were embedded. Robust plans were not always in place to support people to manage risks effectively. Facilities and equipment were not always safe and well-maintained, ensuring risks to people were mitigated. Staff did not always receive appropriate training to carry out their role effectively and safely. Medicines were not always stored, administered and managed safely. The provider took immediate action to make improvements around training and environment we found were required at this inspection. People were not always involved in the planning of their care. Staff told us they enjoyed their roles and could contact leaders if they needed support. People had access to activities and were supported by regular staff.

In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.

We have asked the provider for an action plan in response to the concerns found at this assessment.
