This provider is required to have a registered manager to oversee the delivery of regulated activities at this location. A registered manager is a person who has registered with the Care Quality Commission. Registered managers and providers are legally responsible for how the service is run. At the time of our inspection there was no registered manager, though a manager was in post who was applying to become registered.

During this assessment we found the provider failed to achieve the necessary standards and the rating has remained requires improvement. We identified breaches of regulations in relation to safe care and treatment, consent, governance, and staffing.

Consent records were not always in place or not always being consistently completed. We found concerns relating to safety of the environment and peoples risk management. People’s care records were not always detailed or completed consistently. Quality assurance and governance systems were not as effective as they should be. Staff training needed to be improved and staff supervisions and appraisals were not regularly taking place. Policies and procedures had not always been followed. However, permanent staff showed an understanding of people and their needs. We received good feedback from staff in relation to the manager.

We have asked the provider for an action plan in response to concerns relating to safe care and treatment, consent and staffing. For the more significant concerns relating to governance, we have told the provider when they need to be compliant by.

  1. Safe

    No rating

Safe - this means we looked for evidence that people were protected from abuse and avoidable harm.

We identified breaches of legal regulations.

Although we did not see any direct impact, the lack of some risk related paperwork suggested that people’s risks were not always being appropriately managed. The environment was not always managed safely. The provider failed to monitor and address concerns around fire safety, health and safety of the premises. The home was clean and tidy.

Recruitment procedures were safe, though staff did not receive regular supervisions or appraisals and staff training needed to be improved. A high number of agency staff were being used to ensure appropriate staffing levels were in place.

The service mostly had safe systems for appropriate and safe handling of medicines. However, people’s moisturising and barrier creams were not stored safely and some containers were not labelled with the person’s name or instructions for use.

When necessary safeguarding referrals were made and following accidents and incidents lessons learned were being completed. Handovers took place at the beginning and end of each shift and were detailed and necessary referrals were being made when healthcare support was required.

  2. Effective

    Good

Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

We identified breaches of legal regulations.

Relatives told us they were involved in the developing of their loved one’s care records. Staff knew people well, but care records needed to be more detailed and although care plans were reviewed they did not always identify inconsistencies we found. People received the support they needed, could access healthcare support when required and clinical support was available for people. People had hospital passports in place should they need to go to hospital. Although people were asked for consent a care task was performed, records relating to consent were not always in place.

  3. Caring

    Good

Caring – this means we looked for evidence that the service involved people and treated them with compassion, kindness, dignity and respect.

People told us staff were kind and caring, though we did witness that some agency staff were not as interactive as they could have been. People were able to make choices and take part in activities, though the activities coordinator was new to post and was trying to drive improvement in this area. For the most part, people’s immediate needs were met. Staff said they felt safe, though they provided mixed feedback about their workload.

  4. Responsive

    Good

Responsive – this means we looked for evidence that the service met people’s needs.

People told us they were well cared for, but we found the delivery of person-centred care could be improved, though people were supported to access appropriate treatment when needed. People were supported to express their views, access information and provide feedback, though feedback was not always analysed. Some improvements were needed to ensure peoples end of life care plans were adequately detailed.

  5. Well-led

    No rating

Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

We identified breaches of legal regulations.

Governance was not always robust and policies and procedures were not always being followed. It was not always clear how learning, improvement and innovation was taking place in relation to service improvements. The service supported a positive culture and shared direction. Staff said they felt supported, though it was difficult to determine how effective the new management team would be due to historic issues. Staff spoke about the manager positively, and felt they were treated fairly and were able to speak up if they had any concerns, although the 2023 staff surveys had not been analysed. Park View Care Home With Nursing worked in partnership with other professionals. Appropriate healthcare professional were being involved when needed.
