Enterprise Care Support Limited is a domiciliary care agency registered with the Care Quality Commission (CQC) to provide personal care and support to people living in their own homes. At the time of the inspection 73 people were using the service.
The service continues to be rated good based on the overall findings of this assessment.
However, we found a breach of the legal regulation in relation notifiable incidents. People had not been harmed, but were placed at risk of harm because the provider had failed to always notify the CQC without delay about significant incidents that had adversely affected the people they supported. We have also made 2 recommendations about the providers management of medicines, governance systems and recording keeping. We have asked the provider for an action plan in response to the concerns found at this assessment.
The service continues to be safe. The registered manager and staff understood their duty to safeguard people from abuse and protect them from harm. Staff were recruited safely and regularly supported the same group of people to ensure they received continuity of care. Staff followed current best practice guidelines regarding the prevention and control of infection. Medicines were well-organised and people received their prescribed medicines as and when they should.
The service continues to be well-led. The provider promoted an open and inclusive culture. There were established oversight and scrutiny systems in place. The managers were keen to continuously improve the agency and recognised the importance of learning lessons when things went wrong. People's concerns and complaints were well-managed. The provider worked in close partnership with external health and social care professionals.

Safe
Good
Based on the findings of this assessment we found the service remains good. This meant people continued to be safe and protected from avoidable harm.
However, although people received their prescribed medicines as and when they should, we found concerns in relation to how some medicines records were maintained. We recommend the provider consider recognised best practice on the safe management of medicines and take action to update their medicines recording and monitoring practices accordingly. You can find more details of our concerns in the evidence category findings below.
The provider had a positive culture of learning lessons when things went wrong. Care plans were based on assessments conducted by the provider and various external health and social care professionals prior to people using the service. People were protected from the risk of harm and abuse. Staff understood their duty to protect people from abuse and knew how and when to report any concerns they had to the registered manager and/or the office based staff. People were cared for and supported by staff who knew how to prevent and appropriately manage risks they might face. People received continuity of care from well-trained and supported staff who were familiar with the individual personal care needs, wishes and daily routines of the people they regularly supported. Staffs suitability and fitness to work as care workers in an adult social care setting had been thoroughly assessed by the provider. Staff followed current best practice guidelines regarding the prevention and control of infection including, those associated with COVID-19.

Well-led
Good
Based on the findings of this assessment we found the service remains good. This meant the service continued to be consistently well-managed and led.
However, we did identify some concerns in relation to the providers understanding of their notifiable incident reporting responsibilities and how they operated their governance systems. During this assessment we found the provider had failed to notify us without delay multiple times about the occurrence of some safeguarding incidents and serious injuries involving people using the service. This had placed people at risk of harm and represented a breach of Regulation 18 (Notification of other incidents) of the Health and Social Care Act 2008 (Registration) Regulations 2009.
We also found the providers established governance systems were not always being operated effectively. This was because they had failed to pick up and/or address a number of issues we identified during this assessment including, failing to always report notifiable incidents to the CQC in a timely manner and maintain accurate medicines records and sufficiently detailed risk management plans. We recommend the provider considers recognised best practice in relation to good governance and take action to update their quality monitoring systems accordingly. You can find more details of our concerns in the evidence category findings below.
The registered manager and office-based staff had the right levels of skills, knowledge, experience to lead effectively. The provider’s culture was positive, open, and honest, with leadership and management that was clearly identifiable and transparent. People's concerns and complaints were well-managed. The provider recognised the importance of learning lessons when things went wrong. The provider worked collaboratively with most external health and social care agencies and professionals to plan and deliver people’s home care packages.
