The provider remained in breach of the legal regulations relating to safe care and treatment and good governance. In instances where the Care Quality Commission (CQC) has begun a process of regulatory action, we may publish this information on our website after any representations and/or appeals have been concluded, if the action has been taken forward.
During the assessment, we identified concerns relating to medicines management, care planning, risk management and governance. People's records were not always accurate, complete or person-centred and did not consistently provide staff with sufficient guidance about individuals' needs, health conditions, risks and support requirements.
We found medication recording errors, inaccurate PRN (medicines prescribed to be taken as and when needed) protocols, and risk assessments that did not fully reflect known risks. The provider's auditing and quality assurance systems failed to identify a number of issues found during the assessment, reducing assurance that risks were being effectively monitored and managed. We found governance arrangements were not sufficiently robust.
We observed kind and caring interactions between staff and people using the service, and there was evidence that people were supported to take part in activities that reflected their interests and preferences. Staffing levels were sufficient to meet people's needs, and staff knew people well, demonstrating a good understanding of their individual needs, preferences and routines.
