The provider was previously rated Requires Improvement. The inspection published in April 2022 found breaches of regulation relating to safeguarding, staffing and good governance. Improvements were found at this assessment, and the provider was no longer in breach of these regulations. This assessment was announced and started with a visit to provider's office on 23 September 2026. The registered manager retained overall accountability for the service's operation and quality. Another manager was responsible for the day-to-day running of the service. The office was located within a day centre managed by the registered manager, which the person attended on a regular basis.

The person experienced effective care because assessments were thorough, strengths-based and focused on what mattered most to them. Staff had an in-depth understanding of the person's needs. Care was regularly reviewed and adapted in response to changes in need, helping the person to maintain their wellbeing, maximise their independence and achieve positive outcomes.

They knew the person exceptionally well and used their detailed understanding of the person's preferences, routines and communication to provide highly personalised support that reflected their wishes, strengths and aspirations. Staff recognised and responded sensitively to the person's verbal and non-verbal communication, ensuring their voice remained central even when they could not communicate their wishes directly. As a result, the person was supported to maintain their independence, make meaningful choices, take positive risks and experience the best possible quality of life. Staff felt highly valued and supported to deliver this approach, creating a caring culture that consistently achieved exceptional outcomes for the person.

Staff and managers worked proactively with healthcare professionals, families and partner organisations to ensure the person received highly personalised support that reflected their needs, preferences and aspirations. The registered manager fostered an exceptionally open, supportive and learning-focused culture where staff felt empowered to speak up, develop their skills and continually improve the support they provided. Feedback from the person, relatives, staff and professionals was actively sought, valued and used to drive improvements. Strong leadership, effective governance and a commitment to learning resulted in consistently high-quality care and positive outcomes for the person. This meant the person benefited from support that was continually reviewed, shaped around what mattered to them and delivered by a motivated and well-supported staff team.

Effective systems were in place to report and investigate incidents, accidents, safeguarding concerns and complaints. Actions were taken and lessons learnt were shared with staff to reduce the risk of recurrence. Risks were appropriately assessed and managed in a way that promoted safety whilst maintaining the person's independence and quality of life. Staff had received relevant training and demonstrated a good understanding of the person's needs, risks and communication. The person's medicines were managed and administered safely as prescribed. The person had an individualised, person-centred care plan which was regularly reviewed. The person's relatives were involved in care planning and reviews. Referrals to healthcare professionals were made when changes in the person's needs were identified and staff worked proactively with a range of health and social care professionals to support positive outcomes.

Systems and processes supported the delivery of safe, person-centred care. The person was supported by a consistent staff team who knew them exceptionally well and used their detailed understanding of the person's preferences, communication methods and non-verbal cues to tailor support around what mattered most to them.
