What inspectors found

From the Care Quality Commission's assessment published 11 December 2024 (assessment visit 3 September 2024). Quoted under the Open Government Licence.

Golden World Care Service is a domiciliary care agency, registered with the Care Quality Commission (CQC) to provide personal care to people in their own homes. At the time of this inspection, 10 people were receiving care from the provider.

The service’s previous inspection, published on 23 June 2022, rated the provider as Requires Improvement due to breaches of legal requirements. These breaches related to safeguarding, risk management, staff training and support, infection prevention and control, medicines, complaints, and governance systems.

This focused inspection was undertaken to check if the provider had made the required improvements. We gave 48 hours' notice of our on-site visit, ensuring the registered manager would be available to support the inspection. Inspection activities took place from 23 to 25 September 2024, with office visits on the first two days.

We reviewed four key questions – Safe, Effective, Responsive, and Well-led – all previously rated Requires Improvement. We found that the provider had addressed most concerns identified in the last inspection, including those related to safeguarding, staff training and support, medicines, complaints, and person-centred care.

However, while governance systems had improved, they were not fully effective. The systems failed to identify new issues with record keeping and accessibility concerning risk management and pre-employment checks for new staff. These issues were discussed with the provider, who addressed them by the second day of inspection.

We found no evidence of harm to people, but the provider’s ongoing failure to identify and rectify record-keeping and accessibility issues indicated a continued breach of regulations regarding governance. Consequently, we have repeated this breach.

Despite these concerns, sufficient improvements were made overall to raise the service’s rating from Requires Improvement to Good.

  1. Safe

    Good

Based on the findings of this assessment we found the service had improved and addressed all the outstanding breaches we identified at their last inspection. This meant people were now kept safe and protected from avoidable harm. We have therefore changed the providers rating for this key question from requires improvement to good.

This was because the provider had followed the action plan they had developed after their last inspection and improved how they now protected people from abuse, assessed and managed risk, prevented and controlled infection, trained staff and managed medicines.

In addition, people continued to receive continuity of care from a dedicated group of competent, well-supported staff who were familiar with their individual care needs, wishes and daily routines.

  2. Effective

    Good

Based on the findings of this assessment we have rated this key question good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this. This was because people's personal care needs were assessed before they received a home care service from this provider which staff used to develop peoples personalised care plans. Staff worked together to deliver and meet people’s needs and wishes in line with legislation and current evidence-based good practice and standards. People were supported to stay healthy and well. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests. The care and support people received was routinely monitored to continuously improve it.

  3. Responsive

    Good

Based on the findings of this assessment we found the service had improved and addressed all the outstanding breaches we identified at their last inspection. We have therefore changed the providers rating for this key question from requires improvement to good.

This was because at this assessment we found the provider had followed the action plan we required them to send us after their last inspection and had improved peoples care and risk management plans to make them person-centred.

In addition, staff continued to effectively communicate and share information with people in a way they could easily understand, treat people fairly and help them understand their equality and human rights.

  4. Well-led

    Good

Based on the findings of this assessment we found the service had improved and partially addressed the outstanding warning notice we issued at their last inspection in relation to how they operated their governance systems. This was because we found the provider had made improvements and met most of the outstanding breaches of regulation we identified at their last inspection.

However, further improvements were still required because we found that although the providers governance systems were being better managed, it had still failed to pick up a number of new issues we identified at this inspection in relation to the way some records were not being appropriately maintained or made immediately available on request. This represented a continued breach of regulation 17 (Good Governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, but not the warning notice.

You can find more details of our concerns in the evidence category findings below.

People were complimentary about the way the service was managed, and how approachable the office-based managers and all the senior and care staff were. The provider promoted an open and inclusive culture which sought the views of people receiving a service, their relatives, and staff working for them. The provider worked in close partnership with external health and social care professionals and agencies to plan and deliver people's packages of care and support.
