The assessment was partly prompted by notification of an incident following which a person using the service died. This incident is subject to further investigation by CQC as to whether any regulatory action should be taken. As a result, this assessment did not examine the circumstances of the incident. However, the information shared with CQC about the incident indicated potential concerns about the management of the risk of people’s continence needs. This assessment examined those risks. We also received information of concern related to safe care and treatment. We also undertook this assessment due to the aged rating. Since the last assessment, a new interim manager had started at the service.
The assessment identified some areas where improvements were needed. These included enhancing the quality and detail of care plan documentation, improving the consistency of some care records, strengthening future care planning, ensuring dietary support and monitoring records were consistently implemented, and reviewing aspects of staffing arrangements and medicines management. However, none of these issues resulted in people being placed at immediate risk, and the management team demonstrated a good understanding of the service's challenges and areas for development.
Overall, people received safe, caring and person-centred support from a staff team that knew them well and was committed to promoting their wellbeing, dignity and independence. Staff were kind and compassionate in their interactions, worked effectively with healthcare professionals and responded appropriately to people's changing needs. Systems were in place to assess and manage risks, support people's health needs and maintain a safe environment.
Leaders were open, transparent and receptive to feedback throughout the assessment. They recognised the areas requiring improvement and took prompt action where concerns were identified. There was a clear commitment to learning and continuous improvement, with plans already in place to strengthen care planning, staff knowledge and record keeping. The overall culture of the service was positive, with leaders and staff sharing a common goal of providing safe, compassionate and person-centred care that supported people to achieve positive outcomes.
