What inspectors found

From the Care Quality Commission's assessment published 10 April 2025 (assessment visit 17 February 2025). Quoted under the Open Government Licence.

Date of assessment: 26 and 27 February 2025. Manor House is a care home that provides personal care for a maximum of 16 people. There were 13 people living at the service at the time of our assessment. This assessment was prompted by concerns we received about the service. Our rating for this service has changed from good to inadequate and the service was in breach of 7 regulations relating to safe care and treatment, safeguarding, consent, safe environments, staffing, fit and proper persons and governance. Systems to monitor the quality of the service were not robust or effective. Risks to people were not managed well. People were at risk of harm relating to skincare, choking, malnutrition, falls and managing people’s behaviours. Care plans lacked clear guidance for staff. Medicines were not managed safely. Staff had failed to recognise and report safeguarding concerns to the local authority and CQC. Environmental risk had not always been identified and were not protected from infection control risks. People’s rights were not protected in line with the Mental Capacity Act, 2005. Staff were not recruited safely and there were not enough staff to care for people. Staff did not have the skills and knowledge they needed to keep people safe. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/or appeals have been concluded. This service is being placed into special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we user our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide. We have asked the provider for an action plan in response to the concerns found at this assessment in relation to breaches of regulations 11,18 and 19.

  1. Safe

    Inadequate

This key question was previously rated good. At this assessment the rating has changed to inadequate. We identified 6 breaches of regulation. This meant people were not safe and were at risk of avoidable harm. People were at risk of not receiving safe care because risks to people’s health and safety were not adequately assessed and action was not taken to mitigate risk. The service did not have effective systems to ensure incidents and risks were analysed effectively and action was taken to prevent re-occurrence. The provider had failed to respond sufficiently to previous incidents of harm. The provider had not always identified or appropriately reported incidents of abuse, neglect and improper treatment. Staff were not complying with the principles of the Mental Capacity Act, 2005 which was leading to unlawful restrictions on people. People were not protected from being cared for by unsuitable staff and there were not enough staff to keep them safe. Staff lacked the skills and knowledge to keep people safe. People were not safe from infection control risks and the environment they lived in was not safe.

  2. Well-led

    Inadequate

At our last assessment we rated this key question good. At this assessment the rating has changed to inadequate. This meant there were widespread shortfalls in leadership. Leaders and the culture they created did not assure delivery of high-quality care. The provider was in breach of legal regulations in relation to good governance. Governance systems were not effective in identifying the concerns we identified during this assessment. The provider and manager did not have the skills, knowledge and understanding they needed to deliver a service that provided safe care and treatment to people living at the service. Whilst staff told us they felt better supported under the new provider, we found the culture of the service was not positive. We observed care that people received was task focused and not personalised. The provider did not have processes to demonstrate they listened to and acted upon concerns. Despite working with partner agencies to make improvements, the provider had not made enough improvement. and the service was not safe for people.
