The service provided safe, person-centred, and inclusive care. Staff were trained, supported, and delivered care which promoted independence, choice, and dignity. Systems for safeguarding, risk management, infection prevention and control, and learning from incidents were in place. Care plans reflected individual needs and preferences, and people benefited from strong partnerships with health professionals and local community groups, enhancing wellbeing and social inclusion. Leadership was approachable and supportive, fostering a culture of openness, continuous learning, and innovation. Staff felt confident to speak up, and initiatives to promote equality, diversity, and inclusion were strong. End-of-life care was well planned and coordinated, with positive feedback from relatives.

However, some areas required further embedding to ensure consistent practice. Repositioning and fluid monitoring records were not always fully completed, and medicine management required stronger clinical oversight to ensure protocols were followed. Governance systems were in place but needed further strengthening to ensure audits, particularly around medicines, and daily practices were consistently applied across the service.

The provider had taken immediate action to address all identified shortfalls, demonstrating responsiveness and commitment to improvement. Continued embedding of these practices is required to maintain standards consistently across the service.
