People told us they felt safe, and we observed kind, respectful interactions between staff and people. Staff understood people well and spoke positively about the culture and leadership, describing noticeable improvements in recent months.

However, concerns were identified in relation to safety and governance. Care plans and risk assessments were often inconsistent, incomplete or out of date, meaning staff did not always have the information required to deliver safe, person‑centred care. Key risks such as diabetes management, falls, wound care, nutrition, capacity and consent, and catheter care were not always sufficiently assessed or reviewed. Environmental checks were inconsistently recorded, and gaps were found in staff training and supervision. Although we saw evidence of learning from incidents, the provider’s analysis did not always result in timely or sustained improvement.

We have identified a breach of regulation in relation to safe care and treatment.

Governance systems required strengthening to ensure accurate care planning, effective oversight, and early identification of concerns. Audits had not consistently highlighted issues we found during the inspection, and some safety-related documentation lacked the depth needed for robust assurance. Despite this, leaders acknowledged the shortfalls and were responsive to feedback, with action already underway to address improvements.

We have identified a breach in relation to governance.

We have asked the provider for an action plan in response to the concerns found at this inspection.
