Magnolia House Residential Care Home is a care home providing personal care for up to 62 people, some of whom were living with dementia. At the time of the assessment, there were 59 peoplelivingat Magnolia House Residential Care Home, all of whom received support with personal care.

The provider was in breach of the legal regulations relating to safe care and treatment, good governance and restrictive practices.

People were positive about the kindness and caring nature of staff, the cleanliness of the home, the range of activities available, and the support they received to access healthcare services. Healthcare professionals also described positive working relationships with the service and responsive communication.

However, the assessment identified a number of concerns that meant people were not always receiving consistently safe, person-centred care. Risks were not always effectively assessed, managed or reviewed, and some incidents had not been reported to relevant agencies. People raised concerns about other people entering their bedrooms, delays in receiving support at busy times, and limited involvement in decisions about their care. Care plans were not always up to date or sufficiently detailed to guide staff in meeting people's needs safely.

There were also concerns regarding staffing levels, staff turnover, medicines management, infection prevention and control, and aspects of the environment. Quality assurance and governance systems were not effective in identifying and addressing these issues, resulting in missed opportunities to improve outcomes and learn from incidents.

While staff felt able to raise concerns; improvements were needed to strengthen leadership, oversight, communication, risk management and person-centred care to ensure people consistently received safe, high-quality support.
