We carried out our on-site assessment visit on 23 May 2024, off-site activity started on 24 May 2024 and ended on 2 July 2024 when we held an online feedback meeting with the provider and their consultant. We looked at 13 quality statements.
At our last visit to this service on 9 January 2023, we found people were at risk of avoidable harm because risk management was not effective. The provider had failed to have systems and processes in place to manage risks. This was a breach of regulation 12 (Safe care and treatment). We also found at that inspection that safe recruitment practices were not in place which was a breach of regulation 19 (fit and proper persons employed). The third breach at the last inspection was a breach of regulation 17 (Good governance) because the provider’s quality assurance systems and processes were not effective and had not enabled them to assess, monitor and improve the quality and safety of the service.
At this assessment we found some improvements had been made, however, there was further work to do, and the provider remained in breach of regulation 17 (good governance).
-
Safe
No rating
At our last inspection the provider had failed to fully assess the risks relating to the safety and wellbeing of people and the environment. This assessment showed there were improvements to the environment and systems to maintain a safe environment. Improvements were still needed to ensure a proactive approach linked to best practice and innovation and to ensure risks were adequately assessed and planned for.
-
Effective
No rating
Care records were very task led and did not focus on improving outcomes. Improvements were needed to ensure care records were organised and that what actions need to be taken to meet people’s individual needs and when to review were recorded.
The service supported people to access healthcare effectively.
-
Well-led
No rating
The provider's oversight and monitoring systems had improved with some refurbishment and use of equipment to keep people safe. However, processes were not always in place which consistently and independently identified improvements, across the service, linked to innovation and best practice. Governance and oversight processes did not always manage risks to people. This was a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
