This assessment was undertaken to follow up on previously identified breaches of legal regulation in relation to safe care and treatment, person-centred care and good governance. Improvements were found at this assessment, and the provider was no longer in breach of these regulations.

The provider had strengthened systems and processes for risk management, environmental safety, infection prevention and control (IPC), medicines management and falls monitoring. Records and monitoring charts were regularly reviewed and updated to support safe care. The provider-maintained oversight of improvements through a RAG-rated action plan, which clearly tracked completed, ongoing and outstanding actions. Progress was monitored regularly by the provider and registered manager.

Improvements to kitchen cleanliness, food storage and temperature monitoring had also been sustained.

Improvements had been made to ensure care was person-centred. Care plans were written in a respectful and personalised manner and reflected people's current needs. Repositioning records were completed contemporaneously and in line with assessed needs and care plans. Guidance from healthcare professionals was followed, and care plans were reviewed and updated when people's needs changed. People requiring regular weight monitoring were weighed as per care plan, with records maintained accurately within their care records.

Governance arrangements had improved significantly. Leaders had strengthened oversight of service quality, risk management and record keeping through regular audits and monitoring processes. These systems supported the timely identification of issues, delivery of improvements and ongoing assurance.
