Date of assessment: 19 November to 5 December 2024. Lumina Care Wigan & St Helens is a domiciliary care service. Thirty people used the service. An assessment has been undertaken of a specialist service that was registered for use by autistic people or people with a learning disability. At the time of the assessment, the service was not used by anyone with a learning disability or an autistic person. However, we assessed the care provision under Right Support, Right Care, Right Culture, as it is registered as a specialist service for this population group. Our overall rating is good. Some improvements were addressed during our assessment to ensure peoples medicines were recorded robustly and to ensure there was sufficiently detailed guidance for staff to follow for people’s specific medical conditions. Staff were safely recruited and received training appropriate to their role. People received consistent care from staff who knew them well. People were protected from the risk of abuse and systems were in place to support people through personal challenges or changes in their needs. The provider had clear systems in place to ensure the care provided to people was of a high standard. Systems were in place to engage with people and listen to the views of both people who used the service and their families. 1. Safe

    Good

Our rating is good. Some care plans and risk assessments needed to be updated to ensure they contained enough detail to mitigate risks to people’s physical health. Some aspects of medicines management needed to be reviewed to ensure sufficiently robust systems were being followed by care staff. Actions were taken by the manager during the assessment to respond to any shortfalls in these areas. Systems were in place to ensure accidents and incidents which occurred were recorded, reported and appropriately analysed to demonstrate lessons learnt and ensured people were protected from the risk of abuse. Staff understood how to protect people from the risks of infection and how to ensure peoples home environment was safe. Staff were recruited safely. There were enough staff to ensure people’s safety and meet their needs. Staff received appropriate training. This included training to effectively support people with a learning disability and autistic people. The provider’s trainer had experience in supporting people with a learning disability. This meant, should a person from this population group need support in the future then the staff had access to specialist support and knowledge.

  2. Effective

    Good

Our rating is good. People’s needs were fully assessed before they received a service, and people were involved in the development of their care. Records demonstrated when people had consented to Lumina Care Wigan & St Helens providing them care, and assessments of people’s capacity to consent were in place where there were concerns people were unable to consent. Outcomes of such assessments were decision specific in line with the Mental Capacity Act 2005. People were supported appropriately to access the service of other professionals to ensure their physical health needs were met. This included arranging and attending appointments with people when appropriate. Outcomes for people were reviewed on a regular basis with people and family members involved where appropriate.

  3. Caring

    Good

Our rating is good. People were treated with dignity and respect Staffing was consistent, and people were supported by staff who knew them well. We were told how people felt staff treated them with kindness, and examples were shared which demonstrated staff understood the importance of promoting independence in aspects of people’s care. Some people had additional support to maintain relationships with families and friends and to access activities which were important to them.

  4. Responsive

    Good

Our rating is good. People received a service which was person centred and based upon their personal wishes and preferences in care. Aspects of care, including when people had made decisions about care at the end of their life, were clear for staff to follow and written in a sensitive manner. People knew how to complain and offer feedback about the service. People also knew who to approach to escalate concerns if they needed to.

  5. Well-led

    Good

Our rating is good. The provider had several systems and audit tools in place, which gave them sufficient oversight of the quality of the service. Some would benefit from a review to ensure any shortfalls in the quality of the service and records would be quickly picked up. Information about the service also needed to be reviewed to clearly reflect the specialism of supporting people with a learning disability and autistic people. Staff could explain their individual roles and felt supported by the management team and the provider. Staff told us there was a positive culture, and systems were in place to ensure staff were treated fairly and equitably within the workplace. Staff also felt confident to speak up about any concerns regarding people’s care and felt able to whistle blow on poor practice.
