People consistently received safe, personalised and compassionate care that promoted independence, choice and wellbeing. People and relatives told us staff knew them well, listened to what was important to them and adapted support around their individual preferences and routines. Support helped people remain independent, maintain community connections and continue living in their own homes for as long as possible.

The provider demonstrated outstanding practice in several areas. People benefited from highly personalised care, strong partnership working and a culture where learning was embedded throughout the organisation. Leaders used information from incidents, audits, safeguarding concerns, quality assurance activity and feedback to improve practice and strengthen systems. There was evidence that learning had resulted in improvements to medicines governance, incident management, Mental Capacity Act 2005 (MCA) processes, staff competence and quality assurance arrangements.

People also benefited from effective partnership working. Staff worked proactively with healthcare professionals, community organisations, families and other services to ensure support remained coordinated and responsive. External stakeholders consistently described the provider as a trusted partner and reported that communication and collaborative working contributed positively to outcomes for people receiving support.

Leaders were visible, approachable and actively involved in the service. Staff described feeling valued and supported, and there was evidence of strong workforce development, internal progression opportunities and investment in staff wellbeing. Governance systems provided effective oversight of quality, safety and performance and were used to drive improvement across the service.

Overall, the evidence demonstrated a well-led service with a strong commitment to person-centred care, continuous improvement and positive outcomes. People received care that promoted their wellbeing, respected their rights and supported them to live meaningful and independent lives.
