What inspectors found

From the Care Quality Commission's assessment published 4 October 2024 (assessment visit 11 April 2024). Quoted under the Open Government Licence.

Date of assessment 14 April 2024 to 31 May 2024.

Fareni Lifecare Ltd is a domiciliary care agency that provides personal care to adults and older people in their own homes. The assessment was carried out in response to the quality of care records supplied to us as part of the dormancy process. The service came out of dormancy in February 2024. At the time of the assessment, the service was providing care to 8 people, some of who were living with complex care needs. We identified 4 breaches in relation to safe care and treatment, governance, fit and proper persons employed and staffing. This meant people using the service were at risk of harm. The assessment was completed remotely by an inspector. As part of the assessment, we spoke with the registered manager, 1 care assistant, 8 people who use the service and 1 relative.

  1. Safe

    Inadequate

We found 2 breaches of the regulations, as the service was not always providing safe care and treatment to people, and had not checked staff were working safely. The service did not have robust safeguarding processes in place. Care records demonstrated a lack of recognition by the service to identify, review or manage risk. People’s individual risks had not been identified, and thorough risk assessments were not in place to keep people safe. Staff had not received training, and had not been safely recruited. Medicines were not always well managed, and we found medicine errors which had not been identified by the service. Staff administering medicines had not been trained to do so, and their competency had not been assessed. However, people said they felt safe and were happy with the care they received.

  2. Effective

    No rating

Peoples care needs had not been fully assessed, and most people did not have a care plan in place at the time of our the assessment. Care plans in place did not reflect peoples needs, and did not contain sufficient guidance for staff to follow. The provider did not have systems in place to monitor feedback from people, staff and relatives. The registered manager had limited knowledge of how to provide effective care, and staff had not received training around the Mental Capacity Act. However, people using the service told us staff respected their rights and wishes.

  3. Caring

    No rating

Personalised care plans were not always in place, and those in place did not reflect peoples life histories, preferences or needs thoroughly and accurately. The registered manager did not know people using the service, and relied on staff to share information about people with them. People told us staff were kind and caring, and treated them with respect. Staff knew peoples needs well, and people told us they took time to get to know their individual preferences. Staff attended visits at the time people expected, and no visits had been missed.

  4. Responsive

    No rating

Whilst people told us staff supported them in a personalised way, records did not reflect this. People’s wishes and preferences around their care were not consistently assessed or documented. People were not always involved in the creation and review of their care records, however the provider took some action in response to our assessment findings. Most people did not have a care plan in place, however those in place did not fully reflect peoples physical, mental, emotional and social needs, including those related to protected characteristics under the Equality Act.

  5. Well-led

    No rating

We found 2 breaches of the regulations, as the service did not have robust governance arrangements in place and had not recruited staff safely. The service was not being led in a way which promoted a shared direction and continuous improvement. The registered manager lacked effective oversight to monitor the care people received. There were no governance arrangements in place to drive improvement. There were no processes in place to monitor the performance of staff and their practice. Staff had not received any training, and were working unsupervised without necessary pre-employment checks in place. These significant shortfalls had not been identified by the registered manager, however they did take action in response to our assessment process.
