What inspectors found

From the Care Quality Commission's assessment published 3 March 2025 (assessment visit 5 September 2024). Quoted under the Open Government Licence.

Premium Home Care Services is a domiciliary care service that at the time of this assessment was supporting 65 people in their own homes with the regulated activity of personal care. Not everyone using the service received personal care. CQC only inspects where people receive personal care, which is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.  We carried out this assessment between 30 September 2024 to 10 October 2024. The inspection team included, 3 inspectors, and an Expert by Experience. An Expert by Experience is a person who has personal experience of using or caring for someone who uses this type of care service. We received feedback from other healthcare professionals in respect of the service. During the assessment process, we spoke with 5 people and 11 relatives. This was a responsive assessment due to historical ratings and no registered manager in post. We assessed a total of 16 quality statements from the safe and well-led key questions and found areas of both good practice and concern. The scores for these areas have been combined with scores based on the key question ratings from the last inspection. Though the assessment of these quality statements indicated some areas of good practice, we rated this service as requires improvement as it did not meet legal requirements relating to the safe administration and management of medicines and management oversight and governance. The provider failed to review and monitor people at risk, effectively investigate incidents, accidents, medicine related concerns and identify learning from incidents. The provider did not have effective oversight of the safe management of medicines. The provider did not have effective systems in place to ensure oversight and quality of the service.

  1. Safe

    No rating

Medicines were not always managed to protect people from the risks associated with them. There was a lack of guidance for the safe administration of medicines, lack of oversight and best practice was not always followed. This resulted in a breach of Regulation 12 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. You can find more details of our concerns in the evidence category findings below. Care records did not always ensure staff had the guidance to support people with their individual risks and health needs. Systems and processes to ensure learning opportunities were not effective. People were supported by staff who understood their roles and responsibilities in order to protect people from abuse and the risk of avoidable harm. The provider ensured infection prevention and control processes were in place and staff followed these accordingly.

  2. Effective

    Good

Staff worked collaboratively with external teams and services. However, care records did not always contain the most up to date information reflective of guidance provided by health and social care professionals.

  3. Well-led

    No rating

The provider had not established and operated effective systems and procedures to assess and monitor the safety and quality of the service. During our assessment of this key question, we found concerns around the lack of provider audits, oversight and leadership of the service which resulted in a breach of Regulation 17 Good governance of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. You can find more details of our concerns in the evidence category findings below. There had been several changes in management, and this had impacted the service. Processes and procedures were ineffective in ensuring continuous learning, innovation, and improvement within the service. The provider lacked effective systems and processes that supported partnership working to collaborate for improvement. There was an open and positive culture within the service where most staff felt valued and respected. The provider had policies in place to support workforce diversity, well-being and flexible working.
