What inspectors found
From the Care Quality Commission's assessment published 21 November 2024 (assessment visit 3 April 2024). Quoted under the Open Government Licence.
Sublime Care is a domiciliary care agency providing personal care and support to people including people with learning disabilities and/or autistic people living in their own homes. At the time of the assessment 64 people were receiving personal care. Assessment activity started on 11 April 2024 and we continued to review information submitted by the provider until 6 June 2024. During the inspection we spoke with the registered manager, the operations manager, a field supervisor, a care coordinator and 5 care workers.
We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. ‘Right support, right care, right culture’ is the guidance CQC follows to make assessment and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it.

Right Support: Despite most people’s experience of care being positive the provider’s processes were not always in line with best practice. Risks to people’s health and wellbeing were not always managed well and the provider did not always make the necessary referrals to external agencies when risks were identified. Right Care: Staff told us and care records showed people received person-centred care and their privacy and dignity was upheld. There were processes in place to assess people’s capacity in line with the MCA. Right Culture: The provider had a range of processes in place to gather feedback from people and staff. There were monitoring and auditing processes in place, however, these were not always effective as they had not identified all the issues we found.
We have identified breaches in relation to risk management, management of medicines and good governance. We will ask the provider for an action plan in response to the concerns found at this assessment.

Safe
No rating
During our assessment of this key question, we found concerns around the management of risks, which was a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Despite generally good practices we identified skin integrity assessments did not consider all known risks. We identified errors in the medicine records and the provider did not always carry out follow-up actions when they identified fire safety risks. Most people told us they received care visits on time from regular staff. However, the electronic call monitoring (ECM) data we reviewed showed there were issues with the scheduling and logging of calls which meant there was a risk people would not get their care visits as planned. The provider’s recruitment processes was not always followed, we found some staff did not have a full employment history. We raised this with the provider and they resolved this shortfall immediately. You can find more details of our concerns in the evidence category findings below.
-

-

-
Responsive
Good
The provider gave people opportunities to feedback about their care. People’s equality and diversity needs were assessed and staff took action to ensure people were not discriminated against.

Well-led
No rating
During our assessment of this key question, we found the provider had failed to effectively assess, manage, monitor and improve the quality and safety of the service, which was a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. There were quality assurance process in place but these were not always effective and not identified/resolved all the issues we found staff files, care plans and risk assessments, medicine records and ECM records. The provider has told us how they will make improvements in some of these areas but we could not be assured they always understood how to make all the necessary improvements for the shortfalls we found. You can find more details of our concerns in the evidence category findings below.
