We have rated the service as Requires Improvement. We found two breaches of the legal regulations in relation to safe recruitment and oversight of the service. Although we identified some areas for improvement in the management of medicines we did not see any evidence of harm.

Pre-employment checks had not been consistently completed. Governance systems and audits had not been used to drive improvement. Records were not always updated to help ensure they captured people’s needs.

However, improvements had been made in some areas since the previous inspection. The service now worked in line with legislation when supporting people who lacked the capacity to make decisions. People were asked for their consent before personal care was provided and staff were person-centred in their approach. Staff now received regular supervision and appraisals although there remained gaps in training.

There were enough staff to ensure people’s safety and meet their needs and preferences. A new manager was in post; they had identified areas for improvement and were working with staff to implement changes where required. Staff were positive about the impact the new manager was having on the service.

We have asked the provider for an action plan in response to the concerns found during this assessment.

  1. Safe

    Good

We identified two breaches of the legal regulations.

Risk assessments were in place but there was limited guidance for staff supporting people who required PEG feeds. Systems for the management of medicines required improvement. Recruitment policies had not always been followed.

However, staff were aware of safeguarding processes and were confident about raising concerns both internally and externally. People were supported in line with legislation covered by the Mental Capacity Act. Records showed mental capacity assessments were completed meaningfully and efforts were made to involve people in decision making processes. There were enough staff to support people with their personal care. Agency staff were used to cover any gaps in the rota.

  2. Effective

    No rating

Assessments were completed before people moved into the service. These were updated regularly so staff were aware of people’s current needs. Where appropriate relatives were involved in care plan reviews. People were asked for their consent before any personal care was provided. Staff were aware of people’s preferences and were person-centred in their approach.

  3. Well-led

    No rating

We found a breach of the legal regulations.

Auditing systems were not robust. Records of incidents were not consistently analysed to help identify patterns and themes. Action plans were not developed in response to identified concerns. However, a new manager had been appointed and had identified areas for improvement. The manager was working with staff to proactively drive improvement in the service. Staff were positive about the changes.
