At our last inspection, this service was rated good. At this inspection we found breaches in regulations relating to person-centred care, safe care and treatment, premises, recruitment and good governance. The rating has changed from good to requires improvement.

Incidents were not consistently recorded and there was no oversight of accidents and incidents to enable managers to identify patterns. Care plans were out of date and risk assessments did not guide staff on how to support people when they were distressed or anxious. Systems for safe recruitment had not always been followed. Improvements were required to the way medicines were managed. Areas of the premises needed updating and redecorating.

There were enough staff to support people during the day. An additional member of staff had been added to the rota to support people at lunch time. There were no records available to evidence staff training had taken place, although staff told us they had completed medicine training and there had been a recent moving and handling training session.

People had enough to eat and drink. They were supported to access other healthcare agencies when needed. New managers were in post and had identified areas for improvement. They were working to develop, introduce and embed new systems to help ensure people were supported safely, their health needs were monitored and any concerns quickly escalated.

We identified breaches in relation to person-centred care, safe care and treatment, premises, governance of the service and safe recruitment.

We have asked the provider for an action plan in response to the concerns found at this assessment.
