What inspectors found
From the Care Quality Commission's assessment published 13 August 2024 (assessment visit 2 January 2024). Quoted under the Open Government Licence.
We carried out this assessment between 22 January and 23 February 2024. We completed inspection site visits on 22 and 23 January 2024. We looked at the following quality statements as part of the assessment, safe and effective staffing, medicines optimisation and safeguarding. Keychange Charity Fair Havens Care Home is a care home that provides accommodation and personal care for up to 23 people. The service specialises in providing care for adults over 65 years of age. At this assessment we found breaches in regulations of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. The registered manager was responsive to our feedback when we raised concerns and immediately put in place an action plan to make improvements and reduce identified risks. The provider’s staff recruitment processes were not robust or in line with requirements. This meant that we were not assured that the provider had always taken appropriate steps to assess candidates experience and suitability to work with people. The systems to manage people’s medicines were not always safe, including systems around storage, ordering, record keeping and care planning. Staff had not always taken proactive action to ensure people had a sufficient supply of medicines, which had resulted in some people not receiving their medicines as prescribed. Staff had received training appropriate to their role. We observed that staff were competent when supporting people with their everyday care needs. Staff told us they did not always feel supported in their role and that they felt unable to raise issues with senior leaders as they were were not always available or approachable. The provider had effective systems in place to identify, record and investigate safeguarding concerns. They had reported safeguarding concerns to relevant authorities and worked to incorporate learning when incidents occurred.

During this assessment we looked at 3 quality statements in the key question of safe. We found concerns in 2 of the 3 quality statements, where breaches of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 were identified. The overall rating for this key question combines scoring from quality statements we looked at during this assessment and quality statements scores in line with findings from our last inspection, where the service was rated good.
People did not always receive their medicines as prescribed, and medicines administration records were not accurately maintained. Care planning around people’s medicines did not always contain person specific instructions or information related to their health and medical conditions. Systems around the storage of medicines were not always in line with best practice. This was a breach of Regulation 12 of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
The provider failed to ensure they completed robust recruitment checks when employing new staff. This increased the risks that unsuitable staff may be employed to work with people. This was a breach of Regulation 19 of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
After our assessment site visit, the registered manager took action to make immediate improvements to their medicines management and staff recruitment processes. They were able to demonstrate how they had reduced the main risks associated with the breaches in regulation detailed above. However, they still required time to evidence they had embedded good practice and were no longer in breach of regulations.
No concerns were found in the quality statement of Safeguarding, which we looked at as part of this assessment. The provider had robust systems in place to help ensure people were kept safe from suffering abuse or coming to avoidable harm. Safeguarding incidents were reported and investigated appropriately.
