During the assessment we identified a lack of awareness among staff and management regarding the risks associated with individuals’ health and support needs. Risks were not consistently recognised or managed in a proactive way to ensure people’s safety. Care records and risk assessments often contained conflicting information and lacked sufficient detail to evidence that medical attention had been sought in a timely manner when people required it.

Staff deployment did not always support people to receive effective responsive care. Monitoring records relating to people’s care lacked information and was not always consistently completed or reviewed or clear about any follow up action taken.

Systems in place to monitor the quality of the service did not always contain sufficient detail or operate effectively to drive improvements at the service. This was due to a lack of oversight from the management.
