What inspectors found
From the Care Quality Commission's assessment published 22 August 2024 (assessment visit 3 May 2024). Quoted under the Open Government Licence.
Ranvilles Nursing and Residential Care Home is a care home registered to provide accommodation, nursing, and personal care. The aim of the service is to provide support and care for people who live with dementia and mental health needs. We completed this assessment due to concerns received from a partner agency in relation to risk management and safe systems, pathways and transitions. The assessment was unannounced and started on 14 May 2024. It ended on 3 June 2024. We looked at 10 quality statements. At our last inspection under our old methodology (report published 2 June 2022), the service was rated as good. At this assessment the rating has remained the same. We identified shortfalls about the governance of the service. We were not assured the training in dementia and mental health was sufficient to provide staff with the skills to provide effective support for people, particularly those with complex mental health needs and people who required 1:1 support. We observed a lack of activity and meaningful engagement for people at times. We also found records about people’s risks needed improvement. The provider had recognised most of these areas for improvement and were working with a consultancy company to make those improvements. However, improvements had not yet been embedded enough to make a positive change to the quality of people’s lives at the time of our assessment. Despite this, relatives provided positive feedback about the care and support people received. Staff told us there was a positive culture in the service and were committed to delivering high-quality, person-centred care for people. Staff enjoyed working at Ranvilles and felt valued for the work they did. The leadership team were open throughout our assessment and took our feedback on board.

Safe
Good
We reviewed 3 quality statements for this key question. The service accepted referrals for people who had very complex needs and people on a discharge to assess pathway (This is when people leave hospital, when safe to do so, and continue their care and assessment in a care home) even though they found it difficult to get support from partners for some of these people. The registered manager told us they were reviewing this and working with partners to address this issue. Risks associated with people’s health conditions and support needs were mostly well managed, but improvements were needed with records. There were a sufficient number of staff on duty and the recruitment of staff was safe. The provider had a range of training in place and most staff told us this was sufficient to equip them for their role. However, we were not assured that the mental health and dementia training was in depth enough and provided staff with sufficient knowledge considering the complex needs of the people they supported. The provider's representative took this seriously and told us they would review this. Relatives told us they thought people received safe care from staff at Ranvilles and risks were managed well.

Caring
Good
We only looked at the quality statement of ‘Treating people as individuals’ in this key question at this assessment. Relatives told us staff understood people’s individual needs and supported them accordingly. The feedback staff provided demonstrated they knew how to treat people as individuals, but our observations identified this was variable amongst the team. Activities were in place but needed to be more personalised to people to enhance wellbeing.

Well-led
No rating
The provider’s governance and quality assurance systems did not always drive enough improvement in a timely way to ensure people always received high-quality care. This was a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Processes did not always support staff to provide person-centred and meaningful care and support. The management team were open and honest throughout the assessment process and were keen to ensure people received safe and high-quality care. Staff told us the service was well-led, they were well supported and treated equally regardless of any protected characteristics. Partners were positive about their working relationship with staff at Ranvilles and felt the joined-up approach ensured good outcomes for people’s health.
