We found a breach of regulation in relation to good governance. We found concerns in relation to the robustness of the governance systems and records. Quality monitoring systems and records needed to be strengthened to ensure any shortfalls would be identified promptly going forward. You can find more details of our concerns in the quality statement findings.

Records were not consistently well-managed across key areas. Risk assessments and medicines records did not always contain sufficient detail to guide staff and ensure safe support. Documentation in relation to mental capacity and consent was inconsistent, with some records failing to show that people were involved in decisions about their care or that the legal processes under the Mental Capacity Act 2005 had been consistently followed. Some employment records also had gaps, including missing pre-employment checks and incomplete work histories.

Staff told us they felt supported by the manager and were provided with supervisions. People were supported to have choice and control and were involved in planning their care. There was a varied programme of group and one-to-one activities in place to support people’s wellbeing and engagement. Staff were kind and caring and had a good understanding of how to meet people’s needs. There was a system in place to allow people to express any concerns or complaints they may have.

We have asked the provider for an action plan in response to the concerns found at this assessment.

  1. Safe

    No rating

Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.

The provider was in breach of legal regulation in relation to records (Good governance). Records in relation to risks associated with people’s care, medicines and safety incidents were not sufficiently comprehensive to ensure staff had all the information needed to keep people safe.

  2. Effective

    Good

Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

  3. Caring

    Good

Caring – this means we looked for evidence that the provider involved people and treated them with compassion, kindness, dignity and respect.

At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people were supported and treated with dignity and respect; and involved as partners in their care.

  4. Responsive

    Good

Responsive – this means we looked for evidence that the provider met people’s needs.

At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s needs were met through good organisation and delivery.

  5. Well-led

    No rating

Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

The service was in breach of legal regulation in relation to Good governance at the service as quality and risk monitoring were not always effective. The provider had not identified all the quality, or risk concerns we found during our inspection. This meant prompt action had not been taken to address shortfalls.
