The new manager had begun to make positive changes to improve the service. These included reducing the use of agency staff and amended shift patterns to improve continuity of care, and meetings with people and their families to aid effective communication.
However, medicines management was not always robust, with inconsistencies in practice, record keeping and assurance processes. There was limited evidence to demonstrate that all staff had the appropriate information and training to support the safe administration of medicines.
Care plans lacked essential guidance, including how staff should manage specific medical conditions. The provider's environmental oversight processes were not always effective in ensuring a safe and well-maintained environment. Audits and maintenance records did not consistently demonstrate that identified actions had been completed.
The service was in breach of legal regulation in relation to the safety of the environment, and the way that medicines were managed. It was also in breach of legal regulation in relation to governance and oversight of the service. We have asked the provider for an action plan in relation to the concerns identified during the assessment.
