We identified shortfalls in relation to the running of the service. This included inconsistent record-keeping and confusing information in people’s care plans. We also found medicines storage was not always following best practice and where action had been taken in response to an incident, management had not carried out thorough checks to ensure that equipment purchased was suitable.

Governance processes were not effective as they were not always identifying shortfalls. Where management used dependency tools to determine staffing levels, these had not always taken into account people’s actual support needs and management were not monitoring staff practice sufficiently. Some incidents and accidents had not been reported to CQC in line with the requirements of registration.

Accidents and incidents were learned from and risks identified for some people were recorded and analysed. People received the medicines they required and people were supported to move into the service through safe transition processes.

Staff felt supported and able to speak up. They received regular supervision and training, although some said they felt they would benefit from some additional training. There was a strong management team in place who worked together well in relation to creating a positive culture within the service.

  1. Safe

    No rating

Safe – this means we looked for evidence that people were protected from abuse and avoidable harm.

At our last assessment we rated this key question Good. At this assessment the rating has changed to Requires Improvement. This meant some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.

The service was in breach of legal regulation in relation to safe care and treatment. We found some shortfalls in medicine storage, understanding of infection prevention and control and inconsistent record keeping.

  2. Well-led

    No rating

Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question Good. At this assessment the rating has changed to Requires Improvement. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

The service was in breach of legal regulation in relation to governance at the service as care plans were not contemporaneous, audits did not always identify shortfalls and actions following incidents were not always following safe practices.
