What inspectors found
From the Care Quality Commission's assessment published 22 April 2024 (assessment visit 6 February 2024). Quoted under the Open Government Licence.
Meadow Grange Nursing Home Limited is a residential home without nursing care for older people, some of whom are living with dementia. At the time of our visit, there were 53 people living at the home. The inspection was prompted in part, due to concerns we received about staffing and medicine management as well as the length of time passed since our last inspection. A decision was made for us to assess and examine those risks. We undertook a focused inspection to review some of the key questions relevant to those areas.

Safe
Good
Safe systems were in place to escalate any concerns to external health professionals, we observed positive partnership working whilst on site.
Staff and leaders took appropriate actions to ensure people were safe, to mitigate any risk, ensuring people were not left at risk of harm. Leaders ensured people were supported in line with their individual care plans.
We observed that all areas of the service were clean and well maintained, and where work was underway for areas being refurbished, these were separated off to limit any impact or noise.
There were enough staff deployed during our visit and staff had received suitable training in order to support people safely.
Staff managed medicine administration in line with safe systems for appropriate handling and storage of medicines.

Effective
Good
The provider had recently introduced a new digital care planning system. This would ensure people’s care plans and risk assessments could be developed with the inclusion of people, their relatives, and staff. This was still being completed for some people with further information being added once collated. One relative told us they were, "aware that there is a care plan in place, but feels sometimes they have to initiate conversations about it, so feels the involvement is not consistent.".
People received their care in line with current standards to achieve effective outcomes. Clinical reviews were being held on a regular basis.
We were told how staffing levels were reviewed when changes were identified in the levels of support people required.
People’s rights for seeking consent and respecting their choices were upheld.

There had been an action plan in place, supporting the transition of the registered manager, the implementation of the digital system, and general improvements which were overseen by the nominated individual.
The service worked in partnership with health and social care partners, collaborating when required to support people to achieve any goals or outcomes.
The provider understood their responsibilities under the duty of candour. There were measures in place to help ensure any concern or shortfall, could be reported and resolved.
