This service supports adults with physical disabilities, dementia and learning disabilities and/or autistic people who are living in their own homes.
We carried out this assessment on 2 and 6 October 2026. This service was previously rated as good. We carried out this assessment to confirm whether the rating of good remains accurate. This report does not provide detailed information on areas where we found practice continues to meet a good standard. Instead, our findings focus on any areas where the service needs to improve or where we found exceptional practice.
People received safe and effective support from enough suitably recruited staff, who had completed relevant and in-depth training. Staff felt well supported and spoke positively about the quality and range of learning opportunities available. People’s needs, preferences and risks were assessed in detail, and care plans gave staff clear, person-centred guidance. Staff involved people in managing risks and supported them to maintain choice, control and independence. Medicines were managed safely, with clear protocols for medicines prescribed as required; people confirmed they received their medicines on time with appropriate support from staff. Staff followed national tools such as MUST (to identify people at risk of weight loss) and Waterlow (to identify the people at risk of skin breakdown), sought advice from GPs when needed and promptly escalated concerns about people’s health. The provider monitored people’s care and outcomes through regular, effective audits, which identified issues and supported improvements. Staff assessed people’s mental capacity appropriately and, where people lacked capacity, recorded best interest decisions.
People were protected from avoidable harm because staff understood risks, followed robust policies and responded appropriately when concerns arose. People and relatives were complimentary about staff’s ability to recognise changes in people’s health and share concerns quickly. Feedback showed staff treated people with kindness, compassion and dignity, knew them well and delivered care in a person-centred way. Detailed care plans supported staff to provide care that reflected each person’s individual needs, preferences and desired outcomes. People were listened to and involved in their care, and their feedback about the support they received was positive. The provider considered people’s individual communication, health and support needs, helping people to have equitable experiences and outcomes.
The service had a positive and open learning culture. Leaders investigated incidents, completed effective audits and used the findings to identify concerns, learn lessons and improve the quality and safety of care. Staff shared a clear commitment to delivering high-quality, person-centred care and spoke positively about their roles and the support they received. One staff member said, “We want to deliver brilliant care and there is no excuse for us not to do that.” Governance arrangements were robust and supported effective oversight of care, staffing, medicines, risks and people’s outcomes. Clear policies provided staff with practical guidance and helped promote consistent practice. The provider demonstrated a strong commitment to improvement and innovation through its bespoke digital system, which captured important information and supported effective joint working, particularly in relation to on-call tasks. The provider had also been innovative in the way they shared communication with staff using an audio staff newsletter which staff could listen to.
