Staff assessed risks and managed these in partnership with people and families, and staff responded promptly to changes in people’s needs, often exceeding expectations. Staff managed medicines safely, care was evidence-based and coordinated with external professionals to ensure continuity, particularly during transitions. The environment in people’s homes was assessed, and staff took actions to reduce risks.
The service was well led by visible, compassionate and values-driven leaders who promoted openness, learning and staff wellbeing. Staff felt supported, able to raise concerns and motivated to deliver high-quality care. Governance systems were used effectively by senior staff to monitor quality and drive improvement, Overall, the service demonstrated a positive, person-centred culture with strong leadership and a commitment to continuous improvement.
