What inspectors found
From the Care Quality Commission's assessment published 29 November 2024 (assessment visit 29 April 2024). Quoted under the Open Government Licence.
Daryel Care Greenwich is a domiciliary care agency providing personal care to people living in their own homes. At the time of the assessment 60 people were receiving personal care. Assessment activity started on 16 May 2024 and we continued to assess information until 12 July 2024. We spoke with the registered manager, the deputy manager, a care coordinator, a field supervisor, the nominated individual and the business partners who had been supporting the provider to make improvements. We sent feedback questionnaires to staff to get their feedback. We looked at 21 quality statements including learning culture, safe systems, pathways and transitions, safeguarding, involving people to manage risk, safe and effective staffing, safe environments, infection prevention and control, medicines optimisation, assessing needs, delivering evidence based treatment, how staff and services work together, supporting people to live healthier lives, consent to care and treatment, independence choice and control, equity in experience and outcomes, shared direction and future, capable and compassionate leaders, freedom to speak up, workforce equality, diversity and inclusion, governance, management and sustainability, partnership and communities, learning, improvement and innovation. For those areas we did not assess we used the ratings awarded at the last inspection to calculate the overall rating. At the last inspection in June 2023, this service was rated inadequate and has been in Special Measures since September 2023. The provider has made improvements and the service is no longer rated as inadequate overall or in any of the key questions and is no longer in Special Measures. The service is now rated requires improvement. Despite some improvements there was a continued breach in regard to good governance due to ongoing issues with the monitoring system of care calls, lateness and missed calls. We will ask the provider for an action plan in response to the concerns found.

Safe
No rating
At the last inspection in June 2023 we found breaches in relation to risk assessments, medicines and staff training. The provider has made improvements in these areas but due to ongoing concerns with staff timekeeping, late and missed calls and the provider’s monitoring processes further improvements are needed. Most people told us they received a safe service, however, the issues with consistency of staff and timekeeping meant some people did not feel entirely safe especially when they received care from different staff. Some people and relatives were also not confident in the skills of some staff around caring for people with dementia. The provider acknowledged however, that further improvements were needed to ensure everyone received a consistently safe and effective service.
-
Effective
No rating
At the last inspection In June 2023 we found the provider did not have a robust process for assessing people’s needs. The provider was also not working in line with the Mental Capacity Act 2005. At this assessment, we found some improvements had been made. The provider had made improvements to their assessment processes, however further improvements were needed to ensure people were consenting to their care and treatment. The provider had also improved the detail recorded about people’s health needs. Staff worked in partnership with other health and social care professionals to share information about risks and support continuity of care.
-
Caring
No rating
At this assessment we have not looked at all quality statements under this key question and we have combined the scores based on the key question ratings from the last inspection in June 2023. Despite many improvements we found many people did not feel completely consulted about their care plan and further improvements were needed to ensure all people had full independence, choice and control. Staff showed a good understanding of their responsibility to offer choices when delivering care.
-
Responsive
No rating
At this assessment we have not looked at all quality statements under this key question and we have combined the scores based on the key question ratings from the last inspection in June 2023. There was a mixed response from people and their relatives about the provider’s processes for gathering feedback. Not all protected characteristics were considered when considering people’s care needs, experience and outcomes. Despite this, staff and leaders were positive about the systems and processes in place to ensure people were treated as individuals.

Well-led
No rating
At the last inspection in June 2023 the provider was in breach of good governance due to widespread issues with safety and quality of care and lack of effective oversight. The provider has made many improvements to the safety and quality of the service. However, the processes for ensuring staff attended to people at the right time was still not robust and people continued to experience late care visits and care from inconsistent staff. Many people receiving care told us the provider needed to make further improvements to the quality and safety of care. The lack of improvement meant a continued breach of good governance.
