What inspectors found

From the Care Quality Commission's assessment published 3 September 2024 (assessment visit 15 April 2024). Quoted under the Open Government Licence.

Pharos Supported Services is a supported living service providing personal care to people living in their homes. Not everyone who used the service received personal care. CQC only inspects where people receive a regulated activity of personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.

We carried out our on-site assessment on 17, 22 and 23 April 2024. During this time, we visited the office location, and 4 supported living properties. During this assessment 32 people were receiving support with personal care in 19 supported living properties across the West Midlands Region. The service supports people with learning disabilities and autistic people.

We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities. ‘Right support, right care, right culture’ is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it.

The assessment was prompted by a review of information we held about the service. We identified 3 breaches of the regulations. We found known risks were not always managed and care plans and risk assessment did not always provide sufficient guidance to support some people safely, this was a breach of regulation 12. We found the provider did not always follow their complaints procedure, this was a breach of regulation 16. The provider also failed to have effective systems to monitor the quality of care, this was a breach of regulation17.

  1. Safe

    No rating

Systems to assess, monitor and mitigate risks to people, were not always robust or effective. Staff had not always been provided with clear guidance on how to safely meet people's individual needs and manage risks. Where incidents and safeguarding concerns had been reported to the provider, lessons had not always been learned or improvements embedded into practice. People were not always protected from the risk of infection. This was a breach of Regulation 12 (Safe care and treatment) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Recruitment procedures were robust, and people were supported by adequate numbers of staff.

  2. Effective

    Good

Systems in place to assess people’s needs were not always effective. People were not consistently at the centre of their care, and care plans to guide and inform staff did not always have the information needed to ensure people received the best outcomes.

  3. Caring

    Good

Staff were kind and caring and we saw some good interactions between people and staff. However, the systems in place did not always ensure people achieved their best outcomes.

  4. Responsive

    No rating

The provider’s systems to ensure people were at the centre of their care were not always consistent or embedded across the service. There was a complaints system in place, but this had not been consistently implemented and followed. This was a breach of Regulation 16 (Receiving and acting on complaints) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

  5. Well-led

    No rating

The provider’s governance and quality assurance systems were not always effective to ensure the delivery of good quality care and support to people. Although a number of audits and checks were completed, this had not enabled the provider to identify and address a number of concerns we found during this assessment. These included shortfalls in the assessment and management of risks to people. This was a breach of Regulation 17 (Good governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

The provider responded to issues raised during the assessment and took some immediate actions. This provided reassurance about people’s safety but demonstrated a reactive rather than a proactive service. It was not evident that high-quality leadership was embedded at all levels.
