Reason for assessment: This was a responsive assessment based on feedback we had received from external bodies. Woodlands Park Care Centre is a care home for older people living with dementia.
During this assessment we found breaches of 3 of the legal regulations relating to safeguarding, good governance and the reporting of certain events to The Care Quality Commission. People were not always protected from the risk of potential abuse. During our assessment we reported concerns to the local authority safeguarding team as the provider’s systems and processes for reviewing and reporting concerns were not always effective. The service had not always learnt from incidents or taken action to fully mitigate the potential risk of harm. Governance systems were not effective in overseeing risk and driving improvement. Some improvements were required to improve recording of daily care notes to demonstrate people had received their planned care. People’s clinical risks were identified and assessed, and staff supported people safely. Staff followed best practice guidance in relation to infection control. People received their medicines as prescribed.
We have asked the provider for an action plan in response to the concerns found at this assessment.

Safe
No rating
We identified 2 breaches of the legal regulations relating to safeguarding people from abuse and a failure to notify the Care Quality Commission of certain events, in particular, when the local or funding authority had investigated potential abuse.
We found people had been exposed to potential abuse by other people living at the home. These incidents had been recorded by staff but not all had not been reported to the local authority safeguarding team or the management team. Staff and leaders did not routinely and consistently follow the provider’s policies. For instance, where staff had been hit by people, they did not always complete an incident form. We found lessons were not always learnt from events or signs of distress to prevent a reoccurrence.
However, people told us they felt safe, and this was supported by the views of relatives. The home was clean, and people were supported to receive their medicine as prescribed.

Well-led
No rating
We identified 1 breach of the legal regulations.
Systems were in place to monitor the ongoing quality and safety of the service. However, these were not always effective in ensuring issues from audits were actioned in a timely manner to mitigate risks to people. Records were not consistently well maintained and accurate to promote safe care. We found delays or omissions in reporting of reportable events to CQC and the local authority. Staff and leaders did not routinely and consistently follow the provider’s policies. For instance, where staff had been put at risk of harm by people who were experiencing distress, they did not always complete an incident form.
We received mixed feedback from people and their relatives about the management of the service. Most relatives told us the registered manager maintained good contact with them.
The provider invested in staff and supported them to develop in their role.
