Although the inspection was not prompted by notification of an alleged safeguarding incident involving a person using the service, this incident is subject to further investigation by CQC as to whether any regulatory action should be taken. As a result, this inspection did not examine the circumstances of the incident.

People were supported to maintain relationships and participate in a range of meaningful activities. Relatives spoke positively about communication with the service and felt involved in important decisions affecting their family members.

We found a number of areas of good practice across the service. People were supported to maintain their health and wellbeing through effective healthcare support, safe medicines management and good infection prevention and control practices. Positive partnership working helped ensure people's needs were identified and responded to appropriately.

The inspection identified several areas where improvements were needed. Whilst staff knew people well and delivered caring support, the quality of care planning and record keeping was inconsistent. Records did not always provide sufficient information about people's preferences or the support they required when experiencing distress.

Some recurring incidents involving people experiencing distress had not led to clear changes in care planning or support strategies, limiting assurance that learning was consistently used to improve outcomes.

Despite these concerns, leaders had recognised a number of areas requiring development and had begun taking action to address them. Feedback indicated leaders were approachable and supportive and had fostered a culture that was open to feedback and continuous improvement.
