What inspectors found
From the Care Quality Commission's assessment published 6 September 2024 (assessment visit 24 June 2024). Quoted under the Open Government Licence.
Larkhill Hall is a residential care home providing accommodation and personal care for up to 66 people. The service provides care to older people including those living with dementia. At our last inspection this service was rated requires improvement. We initiated this assessment on 6 August 2024. The assessment was prompted by a review of information we held about the service since the last inspection and to follow up a warning notice we issued as part of our enforcement process. Following this assessment the service remains requires improvement. We identified some improved practice around staffing, learning culture and management oversight. Staff told us they felt the home had improved since the last inspection, and people we spoke with told us they felt safe. However we found concerns around the management of people's medicines and some people's risk assessments which resulted in the service remaining in breach of safe care and treatment. Governance systems had improved and the provider was working towards a detailed action plan. The service needed time to demonstrate that improvements had become embedded over time.

Safe
No rating
We identified a continued breach in relation to safe care and treatment. Medicines records did not always demonstrate people received their medicines safely. We found no evidence people had been harmed and this resulted in a continued breach. You can find more details of our concerns in the evidence category findings below. People told us they felt safe living at Larkhill Hall. The manager understood their role and responsibilities. Staff understood the course of action to take if someone disclosed an allegation of abuse. People felt appropriate action was taken when they raised concerns and feedback. Historically people told us that the concerns they reported had not always been reported on. There was enough trained and qualified staff on shift and staff were recruited safely. Staff knew how to support people and people told us they had been involved in their care and support plans. Lessons had been learnt from previous shortfalls and safeguarding recommendations were being recorded, however, it was not always clear from the record if the recommendations had been implemented.

Well-led
No rating
Staff understood their roles and responsibilities. All reportable incidents and accidents had been escalated and investigated appropriately, and organizational learning had been implemented and communicated to staff during team meetings. Staff told us they could approach the manager if they had any concerns and they had noticed an improvement in the service since the last inspection. Feedback was gathered and analyzed for patterns and trends. The senior managers were aware of their roles with regards to duty of candor, and were open and transparent regarding the previous and remaining issues at the home they were trying to fix. Governance systems, though these were improved and had highlighted some areas of improvement, further work was needed to ensure these systems were imbedded robustly into the service.
