The provider’s systems and processes to manage people's medicines were not always effective. People's care records were not always kept up to date. The provider’s quality assurance audits and checks had failed to identify these issues. We have identified breaches of regulation in relation to safe care and treatment and good governance. We requested an action plan from the provider. Staff were trained and understood most people’s needs so they could provide care to them.
