We found some improvements were required for the management of safety incidents, safeguarding and care plans and risk assessments. Meals were not always in line with the provider’s expectations of quality, and we found some areas of the home were not clean and some kitchen processes not followed. Records were not always accurate, and audits and checks were not always effective at identifying areas for improvement. People had not always had their rights to privacy and dignity respected. Governance systems and audits had not yet shown sustained improvements in the quality and safety of people’s care. Not all relevant events had been notified to the Care Quality Commission.

However, people were involved in their care and supported to work towards achieving their aims and their independence was promoted. There were enough staff to meet people’s needs and medicines practices were in line with good practice guidelines.

We have asked the provider for an action plan in response to the concerns found at this assessment.

This inspection was in response to receiving a number of whistleblowing and safeguarding concerns. Some safeguarding investigations were in progress, and other new incidents identified at the time of our assessment. In addition, one specific incident, previously notified to the Care Quality Commission, whereby a person alleged they were harmed whilst staff assisted them to transfer was subject to a criminal investigation at the time of writing this report. As a result, this inspection did not examine the circumstances of the incident. However, the information CQC received about the incident indicated possible concerns about the management of moving and handling risks. This inspection examined those risks.

  1. Safe

    No rating

Our rating for this key question has now changed to requires improvement.

We identified 1 breach of the legal regulations.

Processes to ensure a learning culture had not yet been fully embedded. People had not always been protected from potential abuse and injury and not all safety incidents had been referred to the local authority safeguarding team. Some areas of the home were not cleaned effectively and not all steps to ensure good infection prevention and control had been taken. However, people’s needs were assessed, and they were helped to achieve their goals and aims. Processes were in place to help manage medicines in line with good practice. There were enough staff to meet people’s needs. Plan to refurbish the environment were underway.

  2. Effective

    Good

Our rating for this key question has remained good.

People were involved in their care planning and supported towards their goals and aims. People had not always been given meals in line with the provider’s own expectations of quality. Risk assessments did not always reflect risks to people and how to reduce those.

  3. Caring

    No rating

Our rating for this key question has changed to requires improvement.

We found 1 breach of the legal requirements.

During our assessment a serious incident occurred that involved a significant breach of a person’s privacy and dignity. Partners and staff had mixed views as to whether the people were always supported with respect.

However, people told us they liked the staff that worked at the service, their independence was supported, and their needs met.

  4. Well-led

    No rating

Our rating for this key question has changed to requires improvement.

We found 1 breach of the legal requirements.

The provider was working towards improvements however, these had not yet been fully embedded, and improvements had not yet been sustained. Not all staff felt involved in the changes or felt the leadership team were approachable; as such, not all staff felt confident speaking up if things were not right. Improvements were required to leaders’ oversight of the service to ensure improvements could be identified and embedded. Systems to provide oversight and ensure compliance were not always effective, for example, notifications were not always submitted to CQC when required and records were not always accurate.

Steps to help people make links to their local community were in place and people had access to health care services as needed.
