The provider supported adults aged under and over 65 years with personal care needs and mental health conditions.

We carried out this assessment on 19 October 2026. This service was previously rated as good. We carried out this assessment to confirm whether the rating of good remains accurate. This report does not provide detailed information on areas where we found practice continues to meet a good standard. Instead, our findings focus on any areas where the service needs to improve or where we found exceptional practice.

The provider had systems in place to monitor the quality and safety of the service, including medicine audits, care plan audits and staff competency assessments.

Staff understood their responsibilities for keeping people safe. They knew how to recognise, report and escalate safeguarding concerns and described involving people appropriately. Staff felt able to raise concerns and understood how to escalate these externally when required. Staff also demonstrated awareness of risk assessments and the importance of supporting people to maintain their independence.

There was evidence of learning following incidents. Risk assessments were reviewed following falls, and medicine incidents were followed up through management review, reflective learning, additional training and competency assessments where required. Team meetings were used to reinforce expectations around incident reporting, medicines management, following support plans and accurate person-centred recording.

The provider’s governance systems had identified areas requiring further improvement. This included the quality of communication records and ensuring records clearly reflected the support provided and what people could do independently. Staff were reminded to read people’s support plans, complete incident reports within required timescales and maintain accurate medicine administration records.

Overall, systems for oversight, monitoring and learning were in place and being used to identify areas for improvement.
