What inspectors found

From the Care Quality Commission's assessment published 9 April 2025 (assessment visit 8 January 2025). Quoted under the Open Government Licence.

Date of Assessment 13 February 2025 to 20 February 2025. Cairn Home is a residential care home, providing accommodation and personal care to older adults with a range of support needs, including those living with sensory impairments and dementia. The home can accommodate up to 29 people, with 1 room dedicated to providing respite care. We carried out a comprehensive inspection of the service. At this inspection we found concerns relating to staff training and the provider was in breach of regulations 12 and 17. Additionally, we found that some care plans contained conflicting information about people’s care needs and there was a lack of consideration to the Mental Capacity Act. There were also issues identified in relation to the management of medicines, the monitoring of some people’s weight management and fluid/food intake. Development was also needed around culture and learning at the service. However, we found the service had a friendly and welcoming atmosphere. Staff and management knew people’s needs and there was evidence of staff escalating concerns appropriately between professionals. We saw staff treating people with kindness, respect and dignity and acknowledged people as individuals. One person told us, "Staff are friendly and I describe us all as being one big family." Care plans were individualised and respected people’s lifestyle choices and preferences. People were encouraged to maintain a good social life outside of the service and were supported by staff to achieve this. Enough staff were in place to safely support people. Managers told us there was on-going staff recruitment, to ensure the service had a core staff team in place. The service was clean and homely, the environment supported people with sensory impairments to safely mobilise around the home. The service was in the process of renovating people’s bedrooms and the management team were in the process of modernising their recording systems, including moving to a digitalised system.

  1. Safe

    No rating

Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question Good. At this assessment the rating has changed to Requires Improvement.

This meant some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.

The service was in breach of legal regulation 12 in relation to people’s safe care and treatment, due to a lack of staff training.

  2. Effective

    Good

Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question Good. At this assessment the rating has remained Good.

This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

  3. Caring

    Good

Caring – this means we looked for evidence that the provider involved people and treated them with compassion, kindness, dignity and respect.

At our last assessment we rated this key question Good. At this assessment the rating has remained Good.

This meant people were supported and treated with dignity and respect; and involved as partners in their care.

  4. Responsive

    Good

Responsive – this means we looked for evidence that the provider met people’s needs.

At our last assessment we rated this key question Good. At this assessment the rating has remained Good.

This meant people’s needs were met through good organisation and delivery.

  5. Well-led

    No rating

Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question Good. At this assessment the rating has changed to Requires Improvement.

This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

During this assessment, we found a breach of regulation 17. Some staff training levels were not sufficient. As a result, we were not assured that all members of staff at the service had the right skills and knowledge to deliver safe and effective care. We have requested an action plan from the provider for staff training. There was also not enough efficient auditing over some areas of people’s: medications, nutritional intake, weight monitoring, mental capacity and ability to consent.
