Village Green Care Home is a care home with nursing care for up to 60 people, some of whom live with dementia. The service is a purpose-built premises with 2 floors with single rooms and several communal areas. At our last inspection the service was rated good. At this assessment the rating has changed to requires improvement and there was a breach of Regulations with regard to good governance. The provider had not submitted safeguarding notifications to CQC as required, which reduced external oversight of safeguarding issues. Quality assurance systems had not always identified areas for improvement. This included the use of sometimes unamended generic care planning and risk assessment templates. Care plans were often lengthy, lacked sufficient personalised information and sometimes contained inconsistent guidance. Although staff knew people well and generally delivered safe care, records did not always provide enough information to support new or unfamiliar staff.

Staff received regular training and support. However, they had not received training in positive approaches to supporting people during periods of distress, despite supporting several people who required this. Following the assessment, the provider confirmed plans were in place to address this.

Despite these concerns, the service had many strengths. Staff worked effectively with healthcare professionals and other agencies to support positive outcomes. People experienced joined-up care, smooth transitions between services and good access to healthcare. The provider had strengthened medicines management through the appointment of a new clinical lead and improved admission processes through the introduction of an admissions coordinator. These roles demonstrated a commitment to sustainable improvement.

Staff promoted equality, diversity and inclusion and understood people's individual needs. The service developed meaningful community links and supported people in ways that reflected their cultural, religious and social preferences. Staff consistently promoted people's independence, choice and control.
