What inspectors found

From the Care Quality Commission's assessment published 13 January 2025 (assessment visit 13 August 2024). Quoted under the Open Government Licence.

Wyndham House is a care home registered to provide accommodation and personal care for up to 50 people. The home specialises in the care of older people. The service was also providing day care services to people. At the time of the inspection there were 34 people living at the home. On the day of first site visit there were 2 people attending the service for day care provision. This assessment was prompted by a notification of an incident following which a person using the service sustaining a serious injury. This incident is subject to further investigation by the Care Quality Commission (CQC) as to whether any regulatory action should be taken. This assessment did not examine the circumstances of the incident. The information shared with CQC about the incident, indicated potential concerns about the management of risks at the home and the quality monitoring of these. This assessment initially only examined 2 quality statements, Involving people to manage risks and Governance, management and sustainability. We identified additional concerns during the first site visit, through our observations and talking to staff, people and their relatives. We, therefore, extended the assessment to include the quality statements of Safe and effective staffing, Infection prevention and control and Capable, compassionate and inclusive leaders. The assessment started on 15 August 2024 with an unannounced visit to the service. A second site visit was carried out on 20 August 2024 and the assessment concluded on 4 September 2024. During the assessment we spoke with 6 people who lived at the home, 3 people’s relatives, 5 members of staff, an agency staff member, one visiting healthcare professional, a member of the provider’s estates team, 1 chef, 1 member of the kitchen team and 1 member of the housekeeping team. The registered manager, deputy manager, quality support manager, operations director and a peripatetic manager were available throughout the site visits.

  1. Safe

    No rating

At the last inspection (publish 31 May 2018) the rating for this key question was Good. At this assessment the rating for this key question has changed to requires improvement. This meant some aspects of the service were not always safe and there was limited assurance about safety. There was an increased risk that people could be harmed.

We found that the service did not effectively implement systems to assess and mitigate risk to the health, safety and welfare of people using the service and the management of their medicines. This was a breach of regulation 12 (Safe care and treatment) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

The provider did not have a robust systematic approach to determine and review the number of staff required in order to meet the needs of people using the service and keep them safe at all times. This was a breach of Regulation 18(1) (Staffing) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

Some improvements were needed in relation to infection prevention and control.

People and their relatives told us they felt safe living at the service and were complimentary about the support received from staff.

The provider took our feedback on board and immediately developed an action plan to address the concerns we identified during the assessment. They have been engaging with CQC to provide updates on the progress made.

  2. Well-led

    No rating

At the last inspection (publish 31 May 2018) the rating for this key question was Good. At this assessment the rating for the key questions has changed to requires improvement. This meant the service management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care. The provider had a variety of quality monitoring systems to support the management team to review and assess the service delivery at Wyndham House. However, we found these were not always effective and robust enough to assess, monitor and improve the quality of care people received. This was a breach of Regulation 17 (Good Governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Staff were mostly positive about the management of the home and the registered manager was positive about the support they received from the provider. The management team and the provider took our feedback on board and started implementing changes during the assessment. Such as addressing the concerns in relation to the carpet and reviewing the risk management related to the open staircase. A focused action plan was developed and shared with CQC and a schedule of regular engagement was proposed to discuss the progress of the actions the provider was taking to address our concerns.
