We carried out this assessment due to concerns we received regarding people’s safety at the service. During our assessment we identified several concerns about people not receiving person centred care, the provider’s compliance with the requirements of the Mental Capacity Act (MCA) 2005, safeguarding and management concerns. The provider was in breach of four regulations, relating to safeguarding, consent, person-centred care and governance.

We were not assured the service provided was always person centred and we found widespread shortfalls in the way the service was managed.

We found safeguarding concerns, including poor oversight of people’s finances and restrictive practices, that were not always appropriately authorised, recorded or reviewed in line with the MCA. Risk assessments and care plans were not always accurate or kept up to date to reflect people’s changing needs.

The service was not always effective in supporting people to maintain or improve their independence. There was limited evidence of structured rehabilitation planning or review of outcomes.

People were treated with kindness, and staff were observed responding to people’s immediate needs. However, personalised care planning was inconsistent, and people and relatives were not always meaningfully involved in decisions about care, goals or restrictions.

Leadership and governance were not effective. The service had experienced significant management change and quality assurance systems were not effective in identifying or addressing known risks. Several issues identified at this assessment had been recognised previously but had not been resolved, increasing risks to people.

At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant the effectiveness of people’s care, treatment and support did not always achieve good outcomes or was inconsistent.

The service was in breach of legal regulation in relation to consent to care and treatment.

  3. Caring

    No rating

Caring – this means we looked for evidence that the provider involved people and treated them with compassion, kindness, dignity and respect.

At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant people did not always feel well-supported, cared for or treated with dignity and respect.
