What inspectors found

From the Care Quality Commission's assessment published 20 November 2024 (assessment visit 2 September 2024). Quoted under the Open Government Licence.

St Antony's is a care home offering personal care and accommodation for up to 12 residents, primarily older adults, most of whom live with dementia and some with mental health needs. At the time of our assessment, the home was at full capacity.

The service was rated 'Good' overall in its last inspection (published 27 January 2018). We conducted this unannounced assessment on 4 September 2024, led by an inspector and an assessor. We reviewed all eight quality statements under the key question, "Is the service Safe." and one statement each under "Is the service Caring." and "Is the service Responsive."

The assessment was triggered by our review of the service’s information, noting that it had not been inspected for over six years. While no evidence of harm was found, we identified two breaches of regulations. These related to failures in maintaining a safe and well-kept environment and in effectively using governance systems to monitor and improve service quality.

Despite these issues, management was already addressing the concerns. Staff demonstrated a clear understanding of safeguarding responsibilities, and there were sufficient staff to meet people's needs. Pre-employment checks were thorough, ensuring staff were fit for the role. Staff were well-trained and experienced, and the premises were clean with appropriate infection control measures in place. Medicines were managed effectively, and people received their prescribed medicines on time.

People were supported to maintain independence and had choice and control over their daily lives, as much as their abilities allowed. Feedback from people using the service was used to improve care, ensuring equality and inclusion.

  1. Safe

    Good

Based on the findings of this assessment, the rating for this key question has changed from Good to Requires Improvement. This indicates that some aspects of the service were not consistently safe, increasing the risk of harm to people.

The concerns centred on how the provider maintained and monitored the safety of the care home environment, particularly fire safety. There were also issues with the provider’s governance systems, including how they identified and addressed problems to improve safety. Some risk management plans were missing, lacked personalisation, or were insufficiently detailed, meaning staff did not always have the necessary information to mitigate risks.

These failures led to breaches of 2 regulations: Regulation 12 (Safe Care and Treatment) and Regulation 17 (Good Governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

We discussed these issues with the registered manager, who acted promptly during and after our visit. They provided additional evidence showing how these concerns were either resolved or being addressed.

More details can be found in the evidence section below.

Care plans were based on thorough pre-assessments conducted by the provider and external health and social care professionals. Staff were knowledgeable about safeguarding, well-trained, and thoroughly vetted before working in the home. Infection control, including COVID-19 measures, followed current best practice guidelines.

  2. Caring

    Good

For those quality statements not inspected, we used the scores awarded at the last inspection to calculate this key questions rating. The overall rating for the service remains good. People were supported to have choice and control and make decisions about how their support was provided. They were encouraged and helped by staff to do as much as they could for themselves, to maintain their independence. People were supported to understand their rights and how the service would make sure these were respected.

  3. Responsive

    Good

For those quality statements not inspected, we used the scores awarded at the last inspection to calculate this key questions rating. The overall rating for the service remains good. People were supported to live in a safe, inclusive environment in which they were treated fairly and free from the fear of being discriminated against. People were supported to understand their equality and human rights and how staff and managers would respect these. Managers made sure staff were given appropriate training and supported to treat people equally and fairly and reduce the risk of them of being excluded from receiving care and support they were entitled to. Managers used people’s feedback to improve care to reduce any barriers people might experience due to their protected characteristics.
