We undertook this assessment in response to information of concern which we had received regarding the management of risks. We found breaches of regulation relating to safe care and treatment and good governance. People were not receiving consistent support to manage risks of choking. Staff deployment meant that some people were not receiving the support they needed in a timely way, including at mealtime and this had a negative impact on people’s dignity. Systems for oversight and governance had not identified the shortfalls we found.

Other risks to people were identified and managed, medicines were administered safely. Staff received the training and support they needed to be effective in their roles. We asked the registered manager to review risks of choking and following the assessment they provided assurances about the actions they had taken to ensure people’s safety.

We have asked the provider for an action plan in response to the concerns found at this assessment.
