What inspectors found

From the Care Quality Commission's assessment published 16 October 2024 (assessment visit 6 June 2024). Quoted under the Open Government Licence.

Linden Manor is a residential care home providing accommodation for persons who require nursing or personal care to up to 28 people. The service provides support to older people, people with dementia, people with physical disabilities and people with sensory impairment. At the time of our inspection there were 26 people using the service. Linden Manor is a care home without nursing care. CQC regulates both the premises, and the care provided, and both were looked at during this inspection. The last rating for this service was requires improvement in the Key areas of Safe, and Well-Led (published 18th January 2024). The service has been rated requires improvement for the last 4 consecutive inspections. We reviewed information we had received about the service since the last inspection. We sought feedback from the local authority and professionals who work with the service. We inspected the areas of Safe, and Well-Led. This inspection was carried out by 2 inspectors, with support from a 3rd.We carried out unannounced site visits on the 17th of June 2024, and the 12th of July 2024, and finished the assessment on 29th July 2024. We saw that improvements had been made from the findings from the last inspection. However further improvements were required around ensuring best practice with best interests, and mental capacity assessments. Not all risk assessments were completed fully and accurately around health needs. Assessments needed to consider people’s long-term independence, be consistent in the information they contain, and follow the provider’s care planning policies and procedures.

We found two breaches of the legal regulations in relation to safe care and treatment and governance.

Staff did not always assess risks to people's health and safety or mitigate them where identified. People did not always have care plans to guide safe practice. Governance systems and audits were not effective in identifying or addressing areas for improvement.

  1. Safe

    No rating

We undertook this assessment as the home required improvement in 2 areas following our last inspection. We had also received information of concern following 2 incidents where people had been at risk of being harmed. We found 2 breach's of the legal regulations in relation to Safe systems, pathways, and transitions, and Infection prevention and control. Measures around medication were not robust. Care plans and audits were not accurate enough to ensure safety and identify areas for improvement. We have asked the provider for an action plan in response to the concerns found at this assessment and how they will embed improvements in practice.

  2. Well-led

    No rating

People, relatives and staff felt the registered manager had a positive impact on the service. We were able to observe the improvements made since our last inspection, but more time was needed to ensure they were fully embedded. The atmosphere and ethos within the service was positive and compassionate, but staff did not always seem clear on their caring roles. Support towards staff who had health needs was good. Ensuring staff training and compliance was thorough. There was an emphasis on working closely with the local community to help to ensure people had enriched lives.

We found 1 breach of the legal regulations in relation to governance and oversight.
